The Maltreatment Assessment Form is the main data collection instrument used for the CIS-2008. It is a three-page standardized form which is completed by the primary investigating child welfare worker at the end of each child welfare investigation.


The purpose of the CIS-2008 Guidebook is to assist workers in completing the form by clarifying questions, answers and providing definitions to questions on the Maltreatment Assessment Form.


The Worker Information Form collects information on all investigating child welfare workers who complete the Maltreatment Assessment Form during the data collection period.


The CIS-2008 Child Maltreatment Assessment Form was adapted to appear as an electronic pop-up form integrated into the client information system in Québec (Projet Intégration Jeunesse {PIJ}). The form appeared as a series of nine tabbed windows following the basic structure of the form.

Participating workers in the CIS-2008 received training whereby they were asked to read the following vignette and complete the child maltreatment assessment form based on the information provided in the vignette. The completed forms were then discussed and discrepancies in responses reviewed to ensure that items were being properly interpreted.
